How to become a patient representative or to be represented on the esveikata.lt portal?
To become a patient’s representative or to be represented on the esveikata.lt portal, you must contact the Customer Service Unit of the State Enterprise Centre of Registers, your general practitioner or create the representation on your own.
1. If you create the representation on your own, you must log in to the esveikata.lt portal using Lithuanian identification means – electronic signature or e-banking, and create a representation of a person by performing the following actions - Patient field→ My representatives→ Create representation and specify representative data → Find→ Select the type of representative →Term of validity→ Save.
2. If you create a representation by contacting the Customer Service Unit of the State Enterprise Centre of Registers, you should submit an application.
You can do this in two ways:
- If you use an e-signature, you must send the signed application by e-mail info@registrucentras.lt, together with the document confirming your right to represent the person.
- If you do not use the e-signature, you must come to the nearest Customer Service Unit of the State Enterprise Centre of Registers and submit an application. You also have to provide your identity document and the document confirming your right to represent the person.
3. If you create a representation by contacting a general practitioner, you do not need to apply to the State Enterprise Centre of Registers. Representation will be created by your general practitioner, to whom you have to provide the representative’s personal data.
Note:
If you have been granted representation, you must accept it, i.e. agree to be a representative. You can do this by logging in to the esveikata.lt portal. On the right side of the screen, at the top, select Patient field → Authorisations granted to me →Accept to be a representative.
